Both psychiatrists (MD/DO) and psychiatric nurse practitioners (PMHNP) are licensed to prescribe psychiatric medications and provide mental health treatment. The main difference is the educational pathway: psychiatrists complete medical school followed by a psychiatry residency, while PMHNPs complete a nursing education and advanced practice specialization in psychiatric mental health.
In Oregon, PMHNPs practice with full independent authority — meaning they can evaluate, diagnose, and treat without a physician's oversight. For most patients, the quality and continuity of the clinical relationship matters far more than the letters after your provider's name.
Your first appointment is a comprehensive evaluation lasting 60–90 minutes. Dr. Perez will ask about your current symptoms, personal and family psychiatric history, medical history, medications, sleep, substance use, and life circumstances. This is a two-way conversation — you should also come with questions.
By the end, you'll have a working diagnosis or differential, an explanation of your treatment options, and a plan you help shape. You won't be rushed.
In most cases, no — you can contact us directly without a referral. However, some insurance plans require a referral from a primary care provider before they'll cover specialty psychiatric care. Check your insurance's requirements before scheduling, or call us and we can help you figure it out.
Telehealth psychiatric appointments are conducted via secure HIPAA-compliant video. You'll need a device with a camera and microphone (smartphone, tablet, or computer) and a private space.
Multiple studies have shown that telehealth is as effective as in-person psychiatric care for most conditions. For patients in rural areas, telehealth often results in more consistent care because distance stops being a barrier to keeping appointments. Prescriptions are sent electronically to your local pharmacy.
We offer sliding scale fees for uninsured and underinsured patients. Oregon Health Plan (OHP/Medicaid) covers psychiatric services — if you think you might qualify, visit OregonHealthCare.gov. We accept select commercial insurance plans.
If cost is a concern, please say so when you contact us. We will always try to find a way to make care accessible — it's a core value of this practice, not a secondary consideration.
No. Medication is one tool among many, and not every patient needs it or wants it. Dr. Perez will discuss the evidence for different treatment approaches — medication, therapy, lifestyle factors, referrals — and work with you on a plan that fits your situation, your values, and your goals.
When medication is recommended, you'll receive a clear explanation of what it does, why it's being recommended, what the expected timeline is, and what side effects to watch for. Nothing is prescribed without your understanding and consent.
